Domain 2-Communicating about the Patient

Oral and Written Communication

Identifying the Problem

  1. Incorporate relevant medical/scientific knowledge
  2. Summarize the essential elements of the medical history
  3. Synthesize the information obtained from physical exam and lab studies
  4. Incorporate any relevant patient preferences

Developing and Maintaining Relationships

  1. Select and adapt information to the audience/reader
  2. Adjust presentation as necessary to meet the needs of the audience/reader
  3. Demonstrate professionalism
  4. Assure continuity of care

Education and Counseling

  1. Facilitate effective group problem-solving
  2. Ensure that care-givers are able to recognize expected outcomes of treatment
  3. Ensure that care-givers are able to manage potential problems

Special Applications

Types-Phone (consult request), case presentation Medical record (H&P, SOAP), consult note,
E-mail response to physician request

Situations-Complication, critical event, emergency transfer to other facility, discharge, death, court testimony
Discharge summary, transfer summary, death summary, death certificate, court deposition.

Team Communication

Identifying the Problem

  1. Ensure that individuals understand their role (as leader or team member)
  2. Ensure that all team members contribute to the understanding of the problem
  3. Assess when the team needs other expertise (e.g., other disciplines, ethics consult,
    legal counsel)
  4. Incorporate new information as it becomes available

Developing and Maintaining Relationships

  1. Assess individual capacity to contribute to the team
  2. Acknowledge contributions of individuals
  3. Focus disagreements on positions, not persons
  4. Provide appropriate support for team members

Education and Counseling

  1. Ensure that all team members collaborate in developing and implementing a plan
  2. Ensure that efforts of the team are directed at the same goals
  3. Negotiate alteration in plan effectively

Special Applications

Types-Work rounds, attending rounds, treatment planning teams

Situations-Negotiating differences in treatment options, negotiating "turf" issues, referrals and consultations, disagreeing with a superior.
